Our services are embedded within our Community Reentry & Wellness Program (CRWP) framework and are designed to ensure timely, integrated access to essential supports across health, social, vocational, and housing domains.

Our Approach to Care Coordination and Case Management

  • Individualized Needs Assessment:
    We conduct thorough, strength-based assessments to understand each client’s goals, barriers, and resource needs, ensuring care plans are uniquely tailored.
  • Integrated Care Planning:
    Clients partner with dedicated case managers and care coordinators to develop a flexible, person-centered service plan addressing health, housing, behavioral health, financial stability, and social connection needs.
  • Collaborative Service Navigation:
    Survivor Sources works closely with healthcare providers, housing agencies, vocational programs, behavioral health specialists, and community partners to coordinate services, reducing gaps and ensuring seamless transitions.
  • Advocacy and Rights Protection:
    We advocate for the rights and dignity of our clients, ensuring equitable access to public benefits, veterans’ services, and culturally competent care.
  • Timely Communication and Documentation:
    Our team maintains clear, timely, and confidential communication with all service providers and stakeholders, including submitting required reports to probation officers, VA representatives, or legal authorities as necessary.
  • Health and Wellness Promotion:
    We actively assist clients in connecting with primary care providers for wellness services, behavioral health treatment, immunizations, and chronic disease management, recognizing health stability as a foundation for life success.
  • Skill Development and Empowerment:
    Through vocational training, personal finance education, and life skills coaching, we support individuals in building confidence, independence, and long-term stability.
  • Emergency Support Access:
    Clients have access to a 24-hour emergency hotline for immediate assistance with urgent needs related to transportation, housing crises, behavioral health emergencies, or health navigation.
